Operation PAWS – Dog House Request Form

Dog houses are available thanks to a generous donation from the Humane World for Animals (formerly Humane Society of the United States(HSUS)).

This program is intended to provide safe, weather-appropriate outdoor shelter for dogs in need within Defiance County. Dog houses will be distributed based on factors including: financial hardship, current shelter provided to the dog, and availability.

Approved applicants will be contacted by phone or email to arrange a site visit, pick-up, or delivery if applicable.

Important: Please complete all sections. Do not submit multiple requests. Duplicate requests may be removed.
